The contact form may collect your name, email address, organization or role, subject, and message. The R.I.S.E. Index™ may collect your name, email address, role, sector, leadership experience, optional identity information, team information, and assessment responses.
How we use information
We use information to respond to inquiries, administer the R.I.S.E. Index™, generate your Leadership Snapshot, support team or facilitated programs you choose to join, improve the experience, and meet legal or safety obligations. Aggregated or de-identified information may be used for reporting, learning, and program improvement.
Forms and service providers
The contact and R.I.S.E. Index™ forms are submitted through Google Forms. When you submit either form, the information is transferred to and processed by Google as a service provider, subject to Google's applicable privacy and security terms.
Information stored in your browser
The R.I.S.E. Index™ stores assessment responses, participant details, and computed snapshot results in your browser's local or session storage so the site can generate and display your Leadership Snapshot. This information stays on that browser unless you submit the form or otherwise share it. You can remove it by clearing this site's browser data.
Sharing and retention
We do not sell personal information. We may share it with service providers that help operate the forms, with authorized facilitators when you participate in a team program, or when disclosure is reasonably necessary for legal, security, or safety purposes.
Information is retained only as long as reasonably necessary for the purposes described here, including program administration, recordkeeping, dispute resolution, and legal compliance.
Your choices

- You may stop before submitting a form.
- You may clear locally stored R.I.S.E. Index™ data through your browser settings.
- You may request access, correction, or deletion of information associated with you by contacting us.
